Job Summary

Position Summary: Working within the context of the client-centered, family focused vision and goals of the Sekw’el’was Home and Community Care Program, the FNIH Home and Community Care Program and in accordance with CRNBC Standards of Practice, the Home Care Nurse combines her/his health sciences knowledge with assessment, supervisory and clinical nursing skills to effectively assess and coordinate client care for the Sekw’el’was Home and Continuing Care Program. The Home Care Nurse provides direction, guidance and supervision of the Home Support Workers.

Qualifications

  • Member in good standing of the College of Registered Nurses of British Columbia (CRNBC)
  • Preferred qualifications Baccalaureate degree in nursing from a Canadian University or recognized equivalent
  • Experience in public health or community-based setting
  • Valid Class 5 Driver’s License and own reliable vehicle
  • Complete Criminal Record Check

  • Knowledge and understanding of First Nations communities, health and social issues
  • Knowledge and understanding of the population health approach and the social determinants of health
  • Excellent written and interpersonal communication skills and the ability to work effectively with a variety of people and circumstances
  • Ability to teach and facilitate learning
  • Ability to guide and supervise Home Support Workers and the ability to establish realistic performance expectations
  • Self-directed with demonstrated organizational skills
  • Tolerance and sensitivity to the needs of every individual and their right to be treated with respect and fairness
  • Ability to maintain a professional demeanour and confidentiality

Responsibilities

  • Participates in the planning, development and evaluation of the Sekw’el’was Home and Community Care Program
  • Coordinate home care services in such a way to meet the home care service needs of the clients while encouraging independence, family and community support
  • Identifies community members requiring home care support
  • Completes a structured client assessment that includes ongoing re-assessment and determines client need and service allocation
  • Develops a client centered care plan
  • Implement the care plan to provide in-home personal care/home management services in collaboration with the Home support Workers & Homemakers
  • Provides mentoring, direction, guidance, support and supervision for Sekw’elw’as Home Support Workers
  • Provides in-home direct nursing care as required
  • Documents the assessments, care plans and service delivery on the clients personal Home Care Health Record
  • Competes and submits service delivery data and reports as required by FNIH Home and Community Care Program
  • Adheres to infection-control guidelines
  • Maintains client confidentiality and ensures that all records are stored securely
  • Provides clients and family caregivers with health information as required through demonstrations and informal teaching sessions
  • Assess the need for capacity building and conducts in-service training sessions for the Home Support Workers and other members of the health team as appropriate
  • Participates in case management and family conferences as appropriate
  • Maintains nursing skills through attendance at workshops, conferences, in-service and staff meeting, in addition to reading current literature and Best Practice Guidelines
  • Ensures that clients admitted to the Home and Community Care Program have access to medical supplies and equipment
  • Maintains professional support and consultation to ensure the program is delivered in a safe effective manner
  • Establishes internal and external linkages with other professionals and community resources
